Chronic kidney disease is common among cardiac surgery patients after their operations, leading to declining kidney function and serious cardiovascular complications. We aim to develop an AI-based decision support model to create a personalized follow-up program for general practitioners. This program will help identify high-risk patients and facilitate early initiation of kidney-protective medications. Our goal is to implement this model nationally and make the algorithm available globally.
